Failure Modes and Effects Analysis (FMEA) helps predict the most likely process failures that will impact a customer and the severity.


To calculate the femea you need to:

  • Identify what can go wrong (the failure) at each step of the process;
  • For each failure, give a number for the severity with a number from 1 (low severity) to 10 (high severity);
  • Determine the probability of occurrence of each failure with a number from 1 (low probability) to 10 (high probability);
  • Determine the probability of finding the failure before the customer with a number from 1 (low probability) to 10 (high probability);

Now you can calculate, for each failure, the Risk Priority Number (RPN) by multiply this 3 number, so you can have a value from 1 to 1000.

Now you can have the failure with more impact (with higher RPN value) and you can assess them for first.

FEMEA is useful in analyze phase to finding issues in existing process and also in the improve phase to look at issue in your solution.
